Our professional staff includes social workers and counselors who can...
- Listen to feelings, thoughts, and plans
- Provide in-home assessment and ongoing mental health services as needed
- Help to understand life/health changes of older adults
- Locate/arrange services to help older adults remain independent
- Provide information/educational presentations for interested groups.

SOP MISSION STATEMENT
The mission of Senior Outreach Program (SOP) is to provide services of a preventive and sustaining nature for
senior citizens whose mental health and well-being are threatened by challenges of the aging process. Extending the commitment of West Iowa Community Mental Health Center, SOP focuses on the coordination and expansion of
existing services to be more accessible to seniors who are isolated by chronic mental health problems and/or a
combination of problems unique to the aging process. The ultimate goal is to maintain the highest possible level of dignity and independence for senior residents
of Crawford County.
